Site Medical Director (Senior & Complex Care) - Signing Bonus

This is a new position that will lead AltaMed PLUS: Senior Premiere Program and will supervise (8) Providers at 3 locations: El Monte, CA (Garvey), Commerce, CA & El Monte, CA (Santa Anita).

* 62 & older age patient group.

Desired candidates will have a minimum of 2 - 5 years experience in Inter-disciplinary Care Planning, SNIF Settings, Hospice, Flexible. Someone who wants to grow professionally. Strategic Vision. Geriatric Experience. Geriatric Fellowship Training. Must have stability & mission aligned.

Specialty: Internal Medicine/ Family Practice

Provider should expect to travel for 4 days with 1 day Administrative Day

Clinic Hours: 8 - 5 PM but needs to be flexible due to being a leadership position.

Minimum Requirements

1. Physician with an active unrestrictive CA license to practice medicine by the Medical Board of California and DEA License required.

2. Prior experience in a leadership or management role strongly preferred.

3. Previous experience with computer-based systems required; Electronic Medical records experience preferred.

4. Education, training and experience necessary to meet the underwritten requirements for inclusion under AltaMed’s malpractice insurance coverage.

5. Current BLS certificate required.

6. Bilingual: English/Spanish/Mandarin/Cantonese preferred depending on location.

1. Experience and expertise in the prevention, diagnosis, and treatment of the illnesses and social problems of the assigned population.

2. Experience and leadership skills in managing medical resources, monitoring the cost effectiveness and quality of services to participants.

3. Experience and leadership in enhancing the patient experience.

4. Ability to work as a team member and participate in the assessment and evaluation process of potential and existing participants.

5. Ability to multi-task and deal with patient care issues in a fast-paced environment.

6. Ability to complete advanced/expert math computations.

7. Strong ability to read, write and speak in a clear, accurate and professional manner; includes active listening skills.

8. Reading and writing skills in the English language are necessary to evaluate patient charts and documents, collect data and understand and give written instructions.

9. Excellent customer service skills and ability to resolve complex customer service issues and exercise conflict management.

10. Proficient in intermediate/advanced computer skills and typing; includes Microsoft Office Word and Outlook.

Physical Requirements:

1. Ability to sit, stand, stoop, reach, bend, and lift up to 10 pounds as necessary to carry out routine duties.

2. Requires travel to sites/programs, special functions or external meetings.

3. Hand and wrist dexterity to utilize computer.

4. May be required to stand for extended periods of computer.

5. Ability to sit for extended periods of computer.

6. May be required to walk for extended periods of time.

7. Audial and verbal capabilities are essential to effectively communicate with patients, clients, and staff.

Environment Conditions Critical to Performance:

1. Exposure to medical clinic setting. Low risk of contaminations when adhering to universal precautions.

2. May have some exposure to outside environments while traveling.

Essential Job Functions:

1. Supervise all medical services offered by the program and provides primary care.

2. Reviews physician and mid-level professional staff performance on quality, patient experience and appropriate resource use at the site no less than monthly, and performs individual provider evaluations no less than annually.

3. Orient all new physician and mid-level providers, and oversee coverage and call schedule.

4. Oversees the management of participant’s medical situations, and oversees the participant’s use of medical specialists and inpatient care.

5. Assist in data collection analysis and preparing reports regarding clinical service delivery.

6. Provide active leadership in the committees to monitor quality of care, patient experience and appropriate resource use indicators and assist in the identification and implementation of appropriate interventions.

7. Ensure that all committees are effectively accomplishing goals established in the QUM Program, and all activities are clearly documented within minutes and/or reports.

8. Collaborates with Quality and Process Improvement and Corporate Medical Management Department to enhance quality of care delivery among all health professional services.

9. Responsible for site coverage in clinic emergencies, and assists in the development of site clinical policies and protocols.

10. Represent AltaMed to external providers and organizations regarding patient management and clinical issues as appropriate.

11. Assists in managing and monitoring the cost of hospitalization, specialty and ancillary services and identifies, implements and measures areas for improvement.

12. Work with Site Physicians to develop coordinated process flows between hospitalist and Physician/Clinic Staff.

13. Work with Site Physician to develop a collaborative process for working with facility discharge planning staff and contracted facilities.

14. Work with Corporate Office Staff to develop system to prepare agendas and attachments for committee meetings, and to maintain accurate, signed, minutes for review.

15. Ensure that chart audits are being conducted, identify problems and provide guidance for resolution. Work through QUMC to audit process and outcomes.

16. Work to develop, implement and monitor clinical guidelines that are appropriate for the assigned population.

17. Work with Site Physician to ensure that pharmaceutical care meets State requirements, and professionally recognized standards of practice.

18. Ensure that effective drug reviews and pharmacy policies are implemented.

19. Ensure that peer review is performed regularly for hospital based providers and clinic based ancillaries.

20. Ensure that contracted pharmaceutical consultant services are provided timely, reports are adequate, appropriate and that there is follow-up.

21. Assist in the development and implementation of the Corrective Action Plans for periodic audits by outside agencies.

22. Performs other related duties as assigned.

Job Type: Full-time

Salary: $208,000.00 - $280,000.00 per year
