I’RAISE + ACS: 

I’RAISE is partnering with the Administration for Children’s Services (ACS) using the School-Based-Early Support model. The goal is to partner closely with schools to connect families to support and services well before concerns warrant a call to the Statewide Central Register of Child Abuse and Maltreatment (SCR). ACS, through I’RAISE, intends to offer families “light touch” service options that strive to connect families to community resources through case management, address families’ concrete needs, support parent/caregiver capacity in their caregiving roles, and avoid unnecessary investigations.

 

Position Summary:

The Case Planner Supervisor will oversee the provision of comprehensive case management services to families enrolled in the School-Based Early Support program. They will lead a team of case planners, working closely with families, school staff, and community partners to assess needs, develop service plans, coordinate access to resources, and monitor progress towards goals. The Case Planner Supervisor will ensure the utilization of trauma-informed practices, motivational interviewing techniques, and evidence-based interventions to support families in achieving positive outcomes.

Responsibilities:

  • Supervise and support a team of Case Planners, providing guidance, coaching, and professional development opportunities
  • Responsible for overseeing 6 Case Planners & 2 Case Aide (1:4 supervisor to case planner ratio)
  • Conduct regular case reviews and provide feedback to Case Planners to ensure high-quality service delivery
  • Collaborate with program leadership to develop and implement policies, procedures, and protocols for the School-Based Early Support program
  • Assist in the recruitment, hiring, and onboarding of new Case Planners
  • Monitor program metrics and outcomes, identifying areas for improvement and implementing strategies for enhancement
  • Serve as a liaison between I’RAISE, ACS, DOE schools, and community partners to promote collaboration and coordination of services
  • Maintain accurate and up-to-date documentation of program activities, including case records, reports, and other documentation as required by funders and regulatory agencies
  • Participate in regular meetings, trainings, and professional development activities

Qualifications / Requirements:

  • Masters of Social Work or an equivalent human services graduate degree as approved by ACS; and
  • At least two years of relevant child welfare experience; and
  • Supervisory experience, although not necessarily in a formal supervisory position.
  • Excellent communication, interpersonal, and organizational skills
  • Proficiency in Google Suite and case management software preferred

Travel Requirement:

  • Ability to travel within New York City public schools to perform job-based duties
  • Must be a New York City Resident with a valid Social Security Number - I’RAISE does not provide relocation assistance nor provide any visa sponsorship at this time
