I’RAISE + ACS: 

I’RAISE is partnering with the Administration for Children’s Services (ACS) using the School-Based-Early Support model. The goal is to partner closely with schools to connect families to support and services well before concerns warrant a call to the Statewide Central Register of Child Abuse and Maltreatment (SCR). ACS, through I’RAISE, intends to offer families “light touch” service options that strive to connect families to community resources through case management, address families’ concrete needs, support parent/caregiver capacity in their caregiving roles, and avoid unnecessary investigations.

Position Summary:

As a Community Pathway’ Worker, you will provide case management rooted in Motivational Interviewing to a maximum of eight (8) families who seek services of their own accord via the program’s community pathway. The community pathway worker would also co-facilitate parent/caregiver support models (EBPs) and support school partnership and outreach efforts with guidance from the School Outreach Liaison.

Responsibilities:

  • Develop and maintain relationships with community organizations, local businesses, and government agencies to enhance access to resources.
  • Conduct outreach activities to raise awareness of available services and programs within the community.
  • Collaborate with stakeholders to identify emerging needs and gaps in services.
  • Provide individualized support to clients, including assessment of needs and goal setting.
  • Advocate for clients to ensure their access to necessary services and resources.
  • Offer guidance and assistance in navigating complex systems such as healthcare, housing, employment, and education.
  • Maintain accurate records of client interactions and progress towards goals.
  • Develop and implement personalized action plans in collaboration with clients.
  • Facilitate referrals to appropriate community services and programs, coordinating follow-up as needed.
  • Respond promptly to crises and emergencies, providing emotional support and practical assistance.
  • Collaborate with emergency services and other relevant agencies to ensure client safety and well-being.
  • Conduct workshops and educational sessions on topics such as financial literacy, health promotion, and life skills.
  • Empower clients to build resilience, self-advocacy, and problem-solving skills.
  • Foster a supportive community environment that promotes mutual aid and peer support.
  • Assist in the development and implementation of new programs and initiatives to meet community needs.
  • Collect data and feedback to evaluate the effectiveness of existing programs, making recommendations for improvement.
  • Work collaboratively with colleagues within the organization and across partner agencies.
  • Participate in team meetings, case conferences, and training to enhance professional development and coordination of services.

Qualifications / Requirements:

  • Bachelor's degree in social work, psychology, counseling, or related field required;
  • Minimum of 2 years of relevant Community Pathway’ Worker/Parent Support (EBP) Co-Lead experience
  • Minimum of 2 years of experience working in a community-based setting, preferably in a direct service capacity.
  • Demonstrated knowledge of community resources and social service systems in New York City.
  • Strong interpersonal skills with the ability to establish rapport and build trusting relationships with diverse populations.
  • Excellent communication skills, both verbal and written, with proficiency in English; proficiency in additional languages a plus.
  • Experience in case management, advocacy, and crisis intervention preferred.
  • Familiarity with trauma-informed care principles and cultural competency best practices.

Travel Requirement:

  • Ability to travel within New York City public schools to perform job-based duties
  • Must be a New York City Resident with a valid Social Security Number - I’RAISE does not provide relocation assistance nor provide any visa sponsorship at this time
